The definition of a Residential Care Facility for Elderly (RCFE) is one which provides non-medical care and supervision to people over the age of 60. Many people will have health conditions that require care and supervision. Community Care Licensing regulates the type of incidental health care that can be provided. For the allowable conditions, certain restrictions apply.
PROHIBITED HEALTH CONDITIONS IN RCFE
RCFEs cannot accept or retain a resident with the following medical conditions. There are some exceptions, which will be designated with an asterisk. Those conditions will require some additional approvals.
- Communicable Tuberculosis
- Dementia * (facilities may be approved to provide dementia care with evidence of provision of safety precautions, a specialized care program and staff training )
- Stage 3 or 4 Decubitis Ulcers (bedsores)
- Gastrostomy Care (G-tube)
- Naso-Gastric Tubes (NG Tube)
- Need for 24 hour Skilled Nursing Care
- Bedridden * (facilities may be approved to provide care for a temporarily bedridden resident, and in some cases longer depending on fire authority approval)
- Staph or other serious infection
- Tracheotomies
- Dependence on staff for all Activities of Daily Living (ADLs)
RESTRICTED HEALTH CONDITIONS IN RCFE
Each has specific requirements, and all require a licensed professional (LVN or RN generally) to develop a care plan, train facility staff on what can and cannot be done by unlicensed staff, and to provide the required care.
Colostomy or Ileostomy - when the area is completely healed
- Licensed professional provides care for ostomy.
- Resident or staff may clean area and change bags.
Contractures - when not severely affecting resident's ability for self care
- Range of motion may be provided by trained staff.
Decubitus Ulcers - Stage 1 or 2, with no open areas
- Licensed professional performs care
Diabetes - including glucose monitoring and insulin
- Resident is able to self test glucose and admininister own medications OR
- Licensed professional does glucose monitoring and medication administration
Enemas, Suppositories or Fecal Impaction Removal
- Resident can do independently
- Licensed professional available to assist
Healing Wounds - when wound is a result of a surgical intervention
- Licensed professional is supervising care
Incontinence - when resident mentally and physically capable of self care
- Incontinence is managed by bowel & bladder retraining or scheduled toileting
- Use of incontinence products if resident can be kept dry & facility odor free
Indwelling Catheter Care - when resident mentally & physically capable of self care except insertion and irrigation
- Licensed professional is available to insert & irrigate catheter
- Staff may change and empty bag
Injections: Intramuscular, subcutaneous, or intradermal only
- Resident may do injections OR
- Licensed professional is available to do injections when ordered (family members are not allowed to perform)
Intermittent Positive Pressure Breathing Machine - when resident mentally & physically capable of operating equipment & determining need OR
- Licensed professional operates and cares for devise
Oxygen - when resident mentally & physically capable of operating equipment & determining their need OR
- Licensed professional can administer an intermittent oxygen order
- Additional safety issues in place, such as signs acknowledging oxygen use, and periodic check of tubing
