  1. Be sure that the facility is licensed by the State of California, Department of Social Services.
  2. Make an appointment on your initial visit to the facility.  This will ensure that a qualified individual will be available to answer your questions.
  3. Have a list of questions regarding facility policies, services for specific health care needs, activities available, etc.
  4. Ask open-ended questions. For example: “If my loved one’s needs change, how will this be handled?”
  5. Spend an adequate amount of time in the facility and speak with both Residents and staff.
  6. Make an effort to either have a meal or observe mealtime at the facility.
  7. Return unannounced to observe mealtime, care services, or activities which your loved one may be interested in.
  8. Ask to see the most updated facility evaluation and licensing reports.  It is required that this is available to you, or you may contact the local Department of Social Services, Community Care Licensing office to obtain this information.
  9. Make an effort to meet with the Administrator or owner of the facility.
  10. Remember that you are still your loved one’s caregiver and primary advocate even when they enter a facility.
